Thank you for your interest in Champions Academy Preschool. Choosing quality care for your child is one of the most important decisions you will ever make. We take your decision very seriously and are committed to living up to the important responsibility of teaching your child. New children are conditionally enrolled at Champions Academy Preschool after their completed registration application and payments are received, subject to the approval of the our corporate office; and will not be considered enrolled and thus can not attend the center, until such approval is obtained.
Step 1: RISK FREE Trial Week
1.
Contact us to setup a trial week
2. Complete Pre-Enrollment Questionnaire and Waiver
3. Pay one week tuition of $72 (2-day trial) or $95 (3-day trial), and get fee credited towards registration if you enroll during the week.
5. Teachers will talk to you, parents, at the end of the trial week to give you necessary paperwork, recommendations, and answer any questions you may have.
6. Take paperwork to front office for Step 2.

Step 2: Enroll in Class
1. Complete and sign Application and AutoPay forms
2.
Pay applicable fees
3. Fill out comments card


Step 3: Attend class on your official starting date
1.
Bring back completed Registration Packet
2. Learn and have FUN!

Enrollment

Annual Registration
$80 non-refundable annual registration fee is due at time of registration.  We do not charge a material fee or book fee.
 
Monthly Tuition (Introductory Rate Valid Until May 31, 2010)
2-day Program      $288/month
3-day Program      $378/month

Registration Information

For your child's safety, we require that all of the forms from your Registration Packet be turned in prior to your child's first day of attendance. Registration Packet includes:

Admission Agreement & Parent Handbook
- Application Form

Pre-enrollment Questionnaire

Emergency Contact Information

Medical Record

In addition we require that you provide an emergency kit for your child in the case of an emergency. Please prepare an emergency kit with the following items:
- Sweatshirt (preferably oversized), pants

Family Photo (for identification purposes)
- Small water bottle
- Index Card with the following information:

Both parent's names and contact information
o  
Out of town contact with phone number
